Medical Records

Need to Request Medical and Imaging Records?

All protected health information (PHI) requests must be in writing.

If you are the patient or the patient's legal representative (parent, guardian, healthcare power of attorney, executor of estate):

1. Download and Complete an Authorization for Use and Disclosure of Protected Health Information.
2. Each blank on the form must be completed. You can enter self on the Name/Title line. You can enter same on the Address line.
3. You will need to furnish a copy of your guardianship, healthcare power of attorney, or executor of estate paperwork.
4. You must sign and date the form.
5. A picture ID is required to pick up records.
Note: If a patient is requesting copies of X-rays, the release form should go to the Imaging Department. If the patient is requesting copies of a medical record, the release form goes to the Health Information Management (HIM) Department. Authorization forms must be submitted to the specific department (Imaging or Health Information Management).

If you are not the patient or the patient’s legal representative:

1. Download and Complete an Authorization for Use and Disclosure of Protected Health Information.
2. Each blank must be completed. Enter the name of the person who will pick up the records on the Name/Title line. Enter the address of the person who will pick up the records on the Address line.
3. The patient must sign and date the form and have a picture ID.
4. Bring the completed and signed form and the required paperwork with you to the medical records department (see hours below).

How Do I Pick Up Medical Records?

A picture ID is required to pick up records.

Copies of Medical Records can be picked up 72(business) hours after request received – Monday thru Friday 8:00 a.m. – 4:00 p.m. The patient can bring the authorization in when the records are picked up, but must notify the Health Information Management (HIM) Department 72 hours (business days) prior to pick up. Records will not be faxed to your home or office. We will fax records to your physician’s office. Call HIM Department at (702) 880-2312.

Copies of X-rays will not be made until time of pickup – Monday thru Friday 8:00 a.m. – 4:30 p.m. The Imaging Department will not prepare or print the patient CD until the patient or representative comes in with a completed authorization form at the time of pickup.
